Here you can register to join the Preferred Provider Network. Registration is free for any funeral home, cemetery, monument company, pet cemetery or other funeral-related business located in the United States. To get started we need the following information for the administrative or main office of your business, as well as the primary contact person. Required fields are noted by an asterisk (*).

If you have multiple service locations, just enter your primary office location at this time. You will be able to submit additional locations after this initial registration is approved.

Administrative Office Location

This is information associated with the main headquarters or coordinating facility of your service and not necessarily that of the physical location. You'll have the opportunity to enter location specific information after your account has been approved.

Contact Information

This info is used for correspondence with the account administrator.

Login Information

This is the information that will be used to set-up your access to the account. Once approved, you'll be able to create additional logins and manage existing logins. Your password must be a minimum of 6 characters with at least 1 letter and 1 number.

Please allow 1 to 3 business days after you submit this form for your registration to be processed. You will receive a confirmation email with instructions for adding more service locations and providing additional information that will appear on your free web pages in our directory.
